Responsibilities

Be a change agent and join the Bureau of Equitable Health Systems (BEHS), a bureau in the NYC Department of Health and Mental Hygiene.   The Bureau of Equitable Health Systems unifies several units to strengthen the Department’s ability to strategically partner with the NYC healthcare system (including but not limited to integrating behavioral health and community linkages into primary care practice). The bureau will engage primary care providers, hospitals, and other healthcare systems to implement evidence-based strategies; leverage information to support planning and technical assistance for providers and payers; advance policy to close the racial equity gap for priority health outcomes; and surface opportunities where health care can influence and connect individuals to social support and address the whole person, beyond physical ailments.

DUTIES WILL INCLUDE BUT NOT BE LIMITED TO:

Specific Duties & Responsibilities:

– Support data operations of chronic disease self-management and prevention initiatives.

– Track and analyze programmatic data, including as needed to inform program priorities and for internal and external reporting.

– Liaise with practice facilitators and program managers on Salesforce patient directory management and quality assurance.

– Identify and address areas of improvement in secure patient information and data management workflows to meet changing program and organizational needs.

– Work with and convene other internal and external stakeholders as needed.

– Assist in execution and management of diabetes, prediabetes, and chronic disease text campaigns.

– Oversee maintenance of accurate and timely information and data on diabetes webpages, liaising with communications teams and other departments as needed.

– Support other departmental program and research initiatives.

– Develop quality indicators to evaluate the process of program implementation to provide real- time, actionable feedback on programs as they unfold and describe and disseminate key findings.

-Lead efforts to develop protocols focused on community engagement, using qualitative and quantitative methods.

– Design, plan, and facilitate trainings, site visits, work groups, and other technical assistance activities provided by the Quality Management unit.

– Propose and implement protocols for reviewing and analyzing performance and quality management data to inform program improvement.

– Provide coaching in quality management and process improvement to organizational partners using quality improvement tools and resources.

Minimum Qualification

1.  For Assignment Level I (only physical, biological and environmental sciences and public health) A master’s degree from an accredited college or university with a specialization in an appropriate field of physical, biological or environmental science or in public health.
To be appointed to Assignment Level II and above, candidates must have:
1. A doctorate degree from an accredited college or university with specialization in an appropriate field of physical, biological, environmental or social science and one year of full-time experience in a responsible supervisory, administrative or research capacity in the appropriate field of specialization; or
2. A master’s degree from an accredited college or university with specialization in an appropriate field of physical, biological, environmental or social science and three years of responsible full-time research experience in the appropriate field of specialization; or
3. Education and/or experience which is equivalent to “1” or “2” above. However, all candidates must have at least a master’s degree in an appropriate field of specialization and at least two years of experience described in “2” above. Two years as a City Research Scientist Level I can be substituted for the experience required in “1” and “2” above.

Appointments to this position are subject to a minimum probationary period of one year.

Salary: $ 75,504.00 - $ 86,830.00
