A New York State Department of Health research team adapted a selected set of core County Health Rankings measures to generate these reports for sub-county level geographies and populations in New York State. Each report provides data for 11 health measures at sub-county levels, including sub-county populations (such as race/ethnicity, age group, Medicaid status, education level) and sub-county geographies (ZIP codes and minor civil divisions where data are available). These reports can be used to assess community health needs, to plan health interventions, and specifically to identify health disparities within counties.
